Their antiinflammatory effects are largely due to suppression of cyclo-oxygenase-2, and most adverse effects, particularly gastrointestinal toxicity, result from inhibition of cyclo-oxygenase-1.3 4 The newer NSAIDs, such as celecoxib, rofecoxib, valdecoxib, and etoricoxib, are highly cyclooxygenase-2 selective.7 Although both selective and standard NSAIDs inhibit cyclooxygenase-2 equally, the real advantage of selective cyclo-oxygenase-2 inhibitors, as suggested by Vane and Warner, is that they are highly cyclo-oxygenase-1 sparing drugs, accounting for reduction in gastrointestinal toxicity by about 50%.3 4 7 These drugs are generally well tolerated, and their clinical efficacy in patients with osteoarthritis or rheumatoid arthritis is comparable to that of non-selective NSAIDs.4 7 8 A recent randomised, double blind, eight day trial comparing etoricoxib 120 mg once daily with indomethacin 50 mg thrice daily in acute gout showed the two drugs to be equally efficacious, with etoricoxib showing an improved safety profile.9 The findings support a potential role for cyclo-oxygenase-2 inhibitors in managing acute gout, and raise important questions. Firstly, are other selective cyclo-oxygenase-2 inhibitors also effective in treating acute gout and other microcrystalline events? This is probably true given the central role of cyclo-oxygenase-2 and prostaglandin E2 in inflammation. Secondly, do the same contraindications and precautions for the use of dual cyclooxygenase-1 and cyclo-oxygenase-2 inhibitors nonselective NSAIDs ; also apply to selective cyclooxygenase-2 inhibitors? Yes--COX 2 inhibitors should be used with caution in patients with cardiac failure, renal insufficiency, hypertension, hepatic dysfunction, peptic ulcer, or on anticoagulants, or with hypersensitivity to NSAIDs. Thirdly, is potential gastrointestinal.

The Legislative Budget Board LBB ; Staff Performance Report to the 79th Legislature recommended the pharmacist be allowed to substitute products not listed in the U.S. Food and Drug Administration's Approved Drug Products with Therapeutic Equivalence Evaluations Orange Book ; . The LBB report estimated that allowing generic substitution of products not listed in the Orange Book would reduce the state's expenditure for prescription drugs for Medicaid by at least .9 million in All Funds for the 2006-2007 biennium. According to the LBB report, approximately .9 million of this amount could be saved for the General Revenue Fund. In May 2005, the Board adopted amendments to the rules implementing the LBB recommendations. Effective June 12, 2005, for drugs listed in the Orange Book, pharmacists may only substitute products that are rated therapeutically equivalent in the Orange Book and have an "A" rating. "A" rated drug products include but are not limited to, those designated AA, AB, AN, AO, AP, or AT in the Orange Book. For drugs not listed in the Orange Book, pharmacists shall use their professional judgment to determine generic equivalency.

Maternal and newborn health services represent a wide range of interventions, depending on whether the mother and newborn are healthy or experiencing problems. The KSPA 2004 draws on the findings and recommendations of Safe Motherhood initiatives such as the Maternal and Neonatal Health Project, promoted by WHO and other international organisations to determine which aspects of maternal health to assess. This chapter uses information obtained in the KSPA 2004 to address the following central questions about maternal health services: What is the availability of ANC? To what extent do facilities have the capacity to support quality ANC services? To what extent is there evidence that health service providers adhere to standards for provision of quality ANC services? 1 To what extent is PNC available where ANC is offered, and to what extent do facilities have the capacity to support quality PNC services? What is the availability of delivery services, and to what extent do facilities have the capacity to support quality delivery services? What are the common newborn care practices in facilities providing delivery services? What is the maternal health provider knowledge on signs and basic interventions of common maternal complications? The objectives of this study were two-fold: 1 ; evaluating carcinogenic potential of a mixture of two persistent pollutants, HCB and PCB126, in an initiation promotion bioassay involving the development of glutathione S-transferase form GSTP ; liver foci; and 2 ; integrating computer modeling with the biology of foci growth in the bioassay. The 8-week bioassay involved initiation with diethylnitrosamine on day 0, two-thirds partial hepatectomy on day 21, and daily oral gavage of HCB + PCB126 mixture from day 14 in young male F344 rats. The animals were sacrificed on days 20, 24, 28, and 56. The mixture treatment significantly increased GST-P foci area and number in the liver, indicating the carcinogenic potential of this mixture. Our biologically based computer model was developed to simulate the occurrence and evolution of initiated GST-P cells in the liver over time. The initiated cells were partitioned into A and B subpopulations with the same division rate but different death rates. Each subpopulation was further categorized into single cells, mini- 2-12 cells ; , medium 13-400 cells ; , and large foci 400 cells ; with different growth kinetics. For the first time, our model quantitatively analyzed the effects of the size of foci on their growth kinetics. The model outputs on relative foci volume, foci number cm3 liver, and size distribution were consistent with the experimentally derived data. This study is part of our continuing development of a predictive tool for carcinogenic potential of chemical mixtures by integrating computer modeling with biology of foci development. The predictive power of such biologically based modeling will increase as more data are available and quantitative correlation can be drawn between chemical properties and critical model parameters.
